CommunityCARE Quality Projects

The DHH CommunityCARE Quality Unit includes 11 staff, with one CommunityCARE Quality nurse in each state region. Goals of the DHH CommunityCARE Quality Unit are:

• To promote healthy behaviors in our Medicaid population resulting in improved health outcomes
  
• To improve medical home management by providing education, office management tools, and utilization data to providers
  
• To develop patient self-care through education
