CommunityCARE is Louisiana's Medicaid Managed Care Program
based on a PCCM Model (Primary Care Case Management)
CommunityCARE is a community-based health care delivery system founded on preventative
and primary health care principles, which focus on the provider/patient relationship.

KIDMED is part of Louisiana's EPSDT (Early Periodic Screening,
Diagnosis and Treatment) Programs.
The goals of this program include early detection and prevention of health conditions.
EPSDT services are available to Medicaid under the age of 21.
